I have the same issue, I think this is a bug, as I also have prisoners who stay in shackles in the cantine. Meanwhile I had perfectly set up cells for the prisoners to live in. Once the holding cell was up the guards briefly escorted the prisoners into the holding cell. I've made this really nice prison with roughly 80 cells. 3) This one is a longshot.
